I check the .php and and there are only 266 lines.

Is it known issue?

BTW, I've installed this on the other site and it works perfectly there.

can you try the following:

change all the short php tags <? to full tags <?php in the php file.

Now I see the map, but without any marks, and before the map I'm getting these errors:

Notice: Undefined offset: 1 in C:\wamp\www\modules\mod_j15visitormap\mod_j15visitormap.php on line 125

Notices are not a problem, you should suppress them in your php, search for suppressing warnings on Google or ask your host.

The marks will come when you start getting hits. It will take a few days sometimes until they start showing up.

is there any specific apache or php module required for visitor map module?

The hosting will need to be able to access the hosip.info site to translate the IP to the location.
